Name Change

My name change occurred after receiving my passport.
I have my original Marriage, Divorce or legal name change document.
My name changed occurred more than one year ago, must have I.D. with new name. Under 1 year, I.D. is not necessary.
Second Valid Passport
I have my current valid 10 year passport.
(If applicable) must submit passport card.
Issuance of Second Valid Passport is at the discretion of the Dept. of State.
Derivative
Original proof of U.S. citizenship for one parent if child was born after February 27, 1983.
If the child was born before that date, both parents need to show their proof of U.S. citizenship.
Proof child was living with both parents.
Or sole custody documents from the court.
Child has Green Card or I551 stamp in foreign passport.
Adoption
Original court documents showing adoption.
Childs original Birth Certificate (translated if necessary).
Original name change documents.
Sex Marker Change
At this time, you can select male (M), female (F), or unspecified or another gender identity (X) as your gender marker.
Passport Refile
I have still not received my passport from the government.
