    Must be signed by applicant ages 16 and over. The parent coming in must sign for child 15 years of age and under.

  • Original Passport– valid for at least six months with at least (2) blank visa pages
  • Visa Application

    Application MUST be typed online, printed and signed. All applicants must sign the application form even if the form is completed by another person. If applicant is under the age 18, a parent or legal guardian may sign on his/her behalf

    All Applicants must complete the China Visa Application Form

    For children

    • The parents of the minor must provide a copy of each parents U.S Passport.
    • If the parents are not U.S. Citizens, they must provide a copy of their status.
    • Copy of the child’s Birth Certificate.
    • If the parents are not accompanying the minor on the trip, both parents must provide a notarized consent letter that states the child has permission to travel and apply for a Chinese Visa.
    • On page 4 of the application, both parents must complete and sign sections 4 and 5. Above the signature at section 4.3 the parents must write the minors name as it appears on the passport.
  • Proof of Travel– Ticket/ E-ticket or Itinerary
  • For Tourism– Hotel Reservation
  • For Foreign passport holders – A copy of the U.S. Alien Resident Card and/or valid visa or document showing legal status in the U.S.
  • One passport sized photo (2×2), with a white background. Photos taken at premises ($15 fee for one set).
  • For Business
    • Business Cover Letter from U.S. company mentioning the applicant’s title/position and detailing exactly what he/she does in the company AND
    • Invitation Letter from China- Must have the seal and signature of the Invitor’s legal representative

    **NOTE: Both letters need to be written on Company Letterhead**

  • Applicants Born in China/Taiwan

    Applicants who were born in China and applying with their US Passport must provide any of the following:

    • Copy of Original Birth Certificate from China (with the Chinese characters)
    • Copy of Chinese Passport or
    • Copy of Taiwanese Passport
  • Consulate Fees

    5-6 Day Service: $150

    **Additional fee for 3-4 day Svc: $30**

    Consulate fee(s) & fee(s) can be combined and paid in CASH/MONEY ORDER (money order payable to “Rush Passport Inc”)
