Visa Application
Applicant can use black or blue ink on the form.
Signed Passport, valid for 6 months or more w/ at least 2 blank visa pages.
Passport Photos
Include two identical passport photos. These must meet the government requirements. 2X2 with a white background, no glasses or head gear.
No Sleeveless Attire

To Prove You are traveling you will need
- Copy of one of the following showing possible travel plans within the next 2 weeks to an international destination. The travel plans must have the applicants name printed on them.
- airline tickets
- reservations
- e-tickets
- cruise tickets
- or itinerary
Business Cover Letter if traveling on business.
Proof of Capacity
Bank Statement, Tax Return or Paystub Showing Sufficient Funds to be used durring your stay in the Philippines
